It definitely is a challenge. No doubt regardless of what insurer you have. BTW, socialized medicine in Canada or the UK is NO BETTER. They have years long wait lists for procedures we get scheduled and done within a week.

I had No Problem with the "Socialized" Medicine in the UK, Worked Out very well for Me and My Family.

A little trick I learned dealing with my own insurance carrier....When you call to dispute a claim, ask what the medical credentials of the adjuster is (90% of the time it's usually just some college intern trained to look for keywords). If they tell you they are not an RN or doctor, ask to speak with the HIPAA compliance officer as your medical records have been accessed by someone who is not a care provider. Your claim will almost always be approved on the spot.
