They should make it necessary to use the check list...like you can't go any further until the BOX or whatever is checked off, so they have someone that can be held accountable...without people being help accountable for deeds, problems like these will just grow and continue. js

What went wrong?! That's easy, they didn't or don't have a check list they should have went thru...wth? Making sure the rider was secure! Someone needs to be fired!
