The "Zero-Defect" Navy (not counting abuse, financial, and sex misconduct) is a problem. A good CO gets fired because his ship went aground on a reef not charted. Or something breaks down below and the ship loses steering causing a collision. How is that the skipper's fault? I just don't see it.
