What Went Wrong in the Smuggler's Notch Avalanche?

SSG Carlos Madden - I remember hearing about this! My dad - after his stint (in North Dakota) during Korea went into the National Park Service & was ski patrol on Mt Rainier. I imagine him looking down from above shaking his head. Conditions were prime.
