Back when I was in as a hospital corpsman we practiced once a year with mass casualty situations and triage just to keep up our skills. I'm not sure if they still continue with that practice.

Same here when I was a National Guard combat medic. We would coordinate with local authorities. The point was not realism in trauma, it was all about logistics.
