Pilot induced loss or mechanical malfunction. Time to break out the Swiss Cheese. That many major mishaps are simply inexcusable. Wonder how many minor mishaps they have that go along with it. Ops tempo high (Crew rest/extended maintenance schedule)? Inexperience Sailors? Recurrent training too low? Perhaps the Navy is putting the wrong Sailors in the cockpit. Many factors are involved in Navy mishaps. The main thing is that our Sailor is safe and no other Shipmates were reported injured.
