About
The Code of Support Foundation (COSF) provides critical one-on-one assistance to service members, veterans and their families (SMVF) in crisis. Through COSF's Case Coordination and PATRIOTlink™ programs we are dedicated to integration of effort to ensure the best outcome for struggling SMVF. Our organizational model uses both human capital and technology to leverage and amplify the assets of service organizations in a collaborative manner. We believe the human element is critical when working with clients and we also understand that the need for effective and efficient cross sector facilitation is equally critical to ensure those positive outcomes. How the organization supports veterans
COSF's Case Coordination Program Case Coordinators (a team of Veteran and Caregiver Peer Navigators) work one-on-one with service-members, veterans and their families to listen, assess and help find the support services they need by leveraging the necessary public and private resources. For a veteran or caregiver in crisis, being able to speak with “one of your own” has proven great success in assisting more than 1,500 struggling veterans and their families so far. We serve all generations of veterans and family members, regardless of era or discharge status. For more information about our Case Coordination program click here.
COSF's PATRIOTlink™ program supports the holistic efforts of our case coordinators. As a result, COSF developed the PATRIOTlink™ tool which is the nation’s first cloud-based veteran resource navigation platform designed by service providers, for service providers. Our intent with the platform (currently in being tested in pilot sites) is to facilitate coordination of resources throughout the country and at all levels of service provision – ensuring those resources are in fact, getting to the troops, veterans and families who would benefit from them.
