This is what I was going to comment as well. It makes sense that with the increasing focus, actual incidents are probably less, although the incidents that are actually identified and dealt with are likely happening at a much higher rate.

I find it is two-fold ... on one hand I think there is a major lack of leadership willing to stop the crap and hold all levels accountable and on the other, more people are reporting it now then prior.

I think this has been an ongoing issue for a long time. However, people are just now starting to understand that it all needs to be reported and leaders need to take it seriously. We have failed to enforce the standards and we as leaders have failed to develop a culture that would discourage these issues from happening.
