70B seems to be loose. Other direct commissions are tied specifically to a degree if not a professional license. When you say medical, what is it that you mean? MSC or a MC/NC?
Go sit down with a medical recruiter and ask.

Thanks for answering LTC Jason. I am willing to get in any medical related position. When I was active duty I was 36B, and currently a 68E dental specialist in the reserve. Also have a full time job work for the VA, so I plan to commission in the medical field if possible. I’m already 33 yr old not sure if I can pass the age limit.

In most cases, but not all, you need a 'Professional" degree in medicine, law or a needed science field.
I know one case where a direct commission was awarded to a veterinarian.
With your background you have a good chance. Not sure about the age issues but good luck to you.
