Has anyone in the Army ever had to go through this process for themselves or one their soldiers? How/where do you begin? Any 4187 examples I can use?

The packets I have seen had a 4187 and supporting documents. I believe among them were acceptance letters for the college or vocational school, the last enlistment contract, and I believe a personal statement. Look at AR635-200 para 5-16 page 57. It lays out requirements to be met which usually drives the supporting documents and narrative portion of the 4187. See your BN or BDE S1. You can't be the first. It goes to the O6 level for approval. I screen them before they go into my BDE Cdr.

Go to the transitions office they will tell you exactly what you need to request it. I picked up a sample packet today for a fellow Soldier.
