Just stopped one of these yesterday with one of our smaller clients. While I put much of the blame on the attacker, the victims aren't blameless. I've worked with so many businesses that look at this from a perspective of "how cheap can I be" or "what's the least I need to do to be compliant" rather than "what can I reasonably do to reduce my risk and protect myself". If they don't put in the appropriate effort and resources to setup preventative controls and to educate their employees, it's almost certain that they will fall for one of these. We all live in the same world, you can't be willfully ignorant of how it works.
