LCDR (Join to see) So, the test equipment was not Calibrated correctly, and no destructive tests were done on a sample of tubes before delivery. Is that correct? Scary stuff if that is the case.

CWO3 Dennis M. it does make one wonder about some of the companies bidding on govt/military contracts.

Having served on two of the older FBM submarines and fired full sized test missiles, this would scare the bejesus out of a crew to know this. Those tubes are huge and failure of one during firing would be catastrophic.

This falls back to previous posts in which I referenced the lack of accountability. When we have civilians involved in Naval shipbuilding operations and repairs. BWX technologies inc. from the beginning was not authorized to perform welding criteria, in accordance with the same test requirements for our bluejackets in the same technical skills set.
This sounds largely like a major brain fart upon our defense spending department and the private sector for not seeking the certifications of all their trades people !
So again I raise my point when in and all levels of leadership are you going to hold everyone to toe. It's not just about embarrassment in the light of things. This involves strategic readiness and the safety of our men and women bluejackets onboard combatants at sea.
I may have retired from the military service, but like my oath there's no expiration date...... I firmly stand by the leadership standards, quality of life, safety and readiness to meet our standards for Naval operations in and out of CONUS. I just can't get my hands around this dropping the ball shit from those men and women in the leadership positions of E-6 and above. Then to the top of the food chain. Senate appropriations committee chairman and everyone in line with the rectal cranial inversion issue !!!

I'll additionally question whether or not they even checked the, welders certifications too verify the qualification dates ? The lack of NDT. AWS, astm and milspec criteria is a absolute indicator of digging deeper into BWX technologies inc. Records ! By now they've probably falsified these in a attempt to cover their ass. One thing for certain, I'd remove this company from any military contract here on out ! I would like to believe that we have a oversight committee, designated to oversee the investigation into the abilities of a government contractor and it's trades personnel staff.
I went through recertification process's, prior to hire even for the Oil field industry which required welding code - ID numbers on every welding process. Thus stated all welding requires a specific certification # ,SMA, GMA, GTA, FLUXCORE, SUBSONIC etc.
Just doesn't add up when senetors/congressman can be set into the armed services committee for military operations, aren't properly vetted for that area of responsibility. You dont just sit behind a desk and start approving government contracts, without a clear cut idea of and what is required to insure that they and their craftspeople hold required credentials which meet our standards per the aforementioned standards.?

LCDR (Join to see) Great post, thanks for sharing. Sounds like only one of three contractors are having an issue so hopefully this wont delay the project too much.
