Sept 26, 1983 in the USSR Lt. Col. Stanislav Petrov disobeys procedures and ignores electronic alarms indicating five incoming nuclear missiles, believing the US would launch more than five if it wanted to start a war. His decision prevented a retaliatory attack that would have begun a nuclear war between the superpowers. A 2014 documentary, “The Man Who Saved the World,” told his story.

I remember the 2007 one, as the commander of the base was relieved because of it. I've heard about all of the others from past reading. A lot of them were equipment malfunctions, and things fail/break.
There will always be transport mishaps, but with the safeguards built into the weapons at least we haven't had a detonation. Knock on wood...
