This is disturbing on many levels. It is clear that primary care AND specialists need to communicate. I believe it also emphasizes that as we age/become chronically I'll/disabled, we need to recognize our limitations on keeping abreast of all our medical needs and we should seek out a caregiver/partner when visiting the doctor(s) and keeping good records of all medical conversations. I do this for my husband, and cc my adult kids as circumstances require, so that we have someone to "follow the thread". Health care personnel are human too, and errors happen. It cannot hurt to have an extra set of ears and eyes at appointments.
