I suspect the Lt. Col. is the "Pilot Error", he needs to keep current to keep his flight pay so he climbs out from behind his Desk-Job and imagines he the same pilot he was decades earlier.. this was my first hand knowledge both flying a T-38 as a student in Del Rio Class 87-08 AND as a Copilot with my crew flying my KC-135A from March AFB to Edwards with a guest Pilot a Major both getting their time in for flight status. They couldn't land the plane - I had to. The Major pranged the landing and I had to push up the throttles and go around and made the landing soo smooth .... and had to roll out and shut down on the taxi off .. the fuel mail was cracked. The T38 guest was a Lt. Col. during a night flight [so he's current] but he could make the runway from the back seat of the T38, I had to. Being said, here is the report:
Two pilots were killed in a crash involving two T-38s at Vance Air Force Base on Nov. 21, 2019. Instructor pilot Lt. Col. John “Matt” Kincade, 47, and student pilot 2nd Lt. Travis Wilkie, 23, were killed in that accident. They were flying on the left wing of a second T-38 to the center runway at Vance, a formation landing, when their aircraft collided with the lead T-38, rolled over top of it and landed inverted off the runway. That accident was blamed on pilot error. <<< which one? ,, yeah the Lt. Col
