This raises the horrible question of how many others are there like this, not only from the AF, but all the military forces? No one knows and very few will step forward on their own to own up. Mad Dog may have ordered an investigation, but given the potential sheer numbers and scope of the screwup, it is unlikely (IMHO) the services will ever come up with identifications and take corrective action. Just a horrible screw-up on the part of some.

Capt Tom Brown It took me a couple of minutes to find his CM paperwork filings. Shouldn't have been hard for them to file them with the Feds. My 2 cents is that there is a mad rush right now in the services to fix this. They do not want another black eye.

Short answer is yes, look no further than everything medical in the military, damn near none of it talks to VA or outside the military. This is a broader issue and something that should be addressed. The question is how do we fix it or what is the systemic issue.

30 % failure rate is a D. They need to examine procedures and take corrective action. A block on a routing sheet attached to the case file would be a start to ensure accurate reporting. Then one last quality check before the file is closed to ensure everything was done vice being pencil-whipped. Vary tragic and sad to lose life, but maybe it will lead to closing this gap.
