Some models can have discrepencies and this looks like the button to charge the coils was pushed, causing the coils to burn and then spark flame. Also maybe his batteries were the wrong kind and overheated. It's happened in the past.

I turn mine off if it goes in my purse or in my pocket. Safety first!
