I just wound up here in the course of my search for information about the (obsolete since Sept. 1976) DA form 3284-R [Applicant's Statement of Name Change"] I filled out & signed, in the course of my Jan. 1973 enlistment process. My recruiter was EXTREMELY EMPHATIC that I'd better be 100% certain that I wanted my 1st name to be Kathy [the name; since, it WOULD be a permanent name change.
