If VA is going to have a clinic then have a full service clinic. Example Bend, clinic in Oregon has doctors, nurses, optometry, audiology. Dont have to travel forever to get those services. CLinic here has no doctors on staff have to teleconf last one from Oregon to Maryland. So they send us out for care and then have to travel or after visit if meds are prescribed hard to get refills thru VA.

I'm not 100% what your comment is pertaining to staffing of providers or if is the lack of specialty care available at the CBOC (Community-Based Outpatient Clinic).
If staffing, then it's a matter of hiring a qualified individual to fill those positions. You can determine the status of open positions by going to https://vacareers.va.gov/ and entering the location.
If the lack of specialty care, it could be that the staffing is also an issue or the the need for that specialty at the CBOC wasn't high enough to warrant a provider (full- or part-time)*.
Regarding the prescriptions - If you're refilling in-person only, then I agree with you. However, the primary method that the VA uses to refill prescriptions is mail-order and you can refill almost all of them either through the VA Health app on your phone, by logging into your account on the VA's prescription refill webpage* and requesting them, or by sending a message to your provider if you don't have any refills left on the prescription.
