In 1992, the Departments of Housing and Urban Development (HUD) and Veterans Affairs (VA) established the HUD-VA Supportive Housing (HUD-VASH) Program. The program is a collaborative effort, supported through HUD Section 8 rental assistance vouchers. The primary goal of HUD-VASH is to move Veterans and their families out of chronic homelessness. A primary component of the program is VA case management, which are designed to improve Veterans' health and mental health, and to enhance their ability to remain stable, housed, and community-integrated. The HUD-VASH program requires continuous coordination with the community's homeless providers and coalitions as well as with other local, state, and federal agencies. Some of the duties include, but are not limited to;
Responsible for coordination of case management services for Veterans receiving housing provided through HUD Section 8 housing vouchers.
Must be clinically knowledgeable to provide and complete assessments with understanding of complex and diverse populations.
Responsible for providing case management for a caseload of Veterans in the HUD-VASH program. The incumbent is responsible for reviewing agreements between Section 8, landlords, Public Housing Authorities (PHAs), and the VA in establishing appropriate referral sites for HUD-VASH placements.
Performs home visits in the community to ensure that Veterans reside in safe environments in compliance with local housing codes.
Conducts community-based visits, made with safety in mind, and follow the Home Visit Safety policy.
Verifies the veteran's status, eligibility for VA medical care, and eligibility of program participation.
Responsible for ensuring that homeless Veterans are considered as primary participants for program acceptance.
Completes all required documents for the HUD-VASH Program including those which are included in the data collection for NEPEC and within HOMES.
Coordinates care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery, and linking and referring Veterans to VA providers, VA Regional Offices, and/or community agencies.
Establishes a continuing relationship with the client, evaluating progress towards goals, performing follow-up after referral, and adjusts treatment plan when needed.
Demonstrates an understanding of the range of treatment and skills needed for all adult patients including geriatric patients related to their physical limitations, psychological needs, and age-associated illnesses and conditions.
Other duties as assigned.
Work Schedule: The primary tour of duty will be M-F 8am to 4:30pm. However, the selectee may also be required to work various shifts on a permanent or temporary basis, based on the needs of the service and patient care.
