The MossRehab Institute for Brain Health (MRIBH), located in the Philadelphia, PA area is recruiting for a full-time Case Manager position. MRIBH, an Avalon Action Alliance network site, is now open and treating Veterans with persistent Traumatic Brain Injury symptoms.

This full-time Case Manager position offers a competitive salary along with excellent benefits.

Responsibilities:

» Assist MossRehab Institute for Brain Health (MRIBH) patients and their families meet biopsychosocial needs by enabling them to access and utilize medical care and community resources.
» Provide medical management coordination to enhance the efficiency of care delivery by improved and prioritized scheduling of services and appropriate allocation of resources.
» This position requires providing service to the following population(s): patients with mild/ moderate traumatic brain injury, Veterans and/ or First Responders.
» Coordinate with inter-disciplinary team.

Qualifications:

» MSW or MSS required.
» BSW may be considered based upon experience and knowledge of care management.
» Position requires 2+ years of clinical experience, preferably in a rehabilitation setting.
» LSW optional.

Preferred Qualifications:

» Experience working with Veterans and/ or First Responders.
» Experience working with patients with brain injury.
