What is the "Reserve Component Individual Acknowledgement of Dwell"?

I am currently working on a packet to submit for a Tour of Duty position and it says that I need to sign a "Reserve Component Individual Acknowledgement of Dwell or Notification of Mobilization form dated 5 February 2014" Has anyone heard of these and know where I get them?

Sir, if you have dwell timing remaining you have to sign a waiver before you can mobilize. Once you sign the waiver you cannot revoke it.

You should be able to use a 4187. Your S1 should be able to provide you the help you need to complete your TOD packet. Also, there should be a POC for every position in TOD that can provide assistance.
