What all is needed for reenlistment and what is the process for it?

Next you figure out what you want to reenlist for. Usually it's a duty station of choice, sometimes training. In your MOS court reporter is something you could request, I think it's ASI C5.
Your career counselor sends the request to HRC and if they can support it, they lock you in for a class seat or an assignment. Then you reenlist. If you picked a duty station you will come on assignment shortly after. If you picked training you will come on assignment 90 days prior to the training start date and attend training TDY enroute to a new duty station

Usually it’s a form your CO signs stating you are good to re up. From there it’s an easy thing, usually an ERB, PT card and the sententious NCO draws up the contract for you to sign. You really don’t have to do anything but have the CO sign, which usually the retention NCO does for you and sign some stuff. Wait for your big day and raise that right hand in front of god and country.

The fact is that your retention ought to handle 100% of the paperwork. Anyway, the form is for the commander to confirm your eligibility for re-enlisting and APFT score card is to make sure you have an up to date one. Remember, a failed APFT is immediate FLAG.
