USMC Conditional Release Process From Active Duty To Army?

I am currently active duty in the Marine Corps as an Air Traffic Controller, and I am in the process of submitting my conditional release to apply for the Army's 153A program. Does anyone have any experience doing this or know of any pointers? I have my DD 368 form complete, but after looking into MCO 1900.16 3004, I have to submit a written request. I have used the template provided but was hoping to find an example of what people are putting for the transfer. I want to make sure I'm not going overboard on anything. Anything helps, thanks!
