How does someone obtain a copy of an Article 15 they received when on active duty?

I would start with your unit commander and then contact HRC to see if it is in your permanent record.

Was it a local completed by your company commander or a field grade? I’m pretty sure a local has no actual paper trail. I know that a GOMOR definitely goes in your restricted file and stays there forever...

It should be in your official military personnel file. This includes everything from the day you enlisted all the way through the fleet. Now I know everyone's moving away from physical records, but it would have been scanned and attached electronically to your military record. I don't know it the Army has something different, but Marines have a system called MOL (Marines On-line) which is where we handle all our personnel related tasks to include requesting leave/liberty, calculating our promotion scores, as well as accessing our personnel record. Every Marine has access to their personnel file. So I would think the Army would have something similar for you as well.

SPC Bradford Bolden - Well, one of 2 things could have happened. Your admin clerks failed in their job since it's their responsibility to upload it. Or 2, maybe someone made it disappear for you. On the plus side, paperwork can't be held against you if it doesn't exist in the first place.

So i know this discussion is over a year old, but some clarifying information is necessary.
Unless you were at least an E-5 at the beginning of the proceedings, it does not get filed with bigger Army. As at least an E-5, it depends on where the imposing commander directs it to go.
Next, in the Army, it is not the clerk's responsibility to file it. It is the legal office's responsibility. It's the clerk's job to enact changes on the systems.
Lastly, if it was not directed to be filed, it is to be destroyed after two years. If you are within that 2 years of when you received the Art 15, a copy could still exist at your last unit's legal office. If they have time, they will usually look it up for toy
