How does one transition from Reserve to Active Duty? Is this a possibility?

It is possible ..... but depends on the needs of active duty at the time and you may loose any bonus that you were given for Reserve membership. With the draw down of active duty forces it is even more challenging to transition - but not impossible. Talk to your local retention NCO; he/she will know the current requirements and likeliness of transitioning.
