How do I get an ID for Transitional Assistance Management Program (TAMP)?

So from my understanding when I separate next Monday i need to get Military ID for TAMP (DD Form 2765) from DEERS so i can still have health care for the next month. But the mil connect site or tricare site doesnt give much info. Does anyone have experience with this?

Are you receiving a discharge below Honorable I.E General or uncategorized? if the answer is yes, then you likely do not qualify for health care, but the best course of action would be to speak to your nearest DEERS office or transition office.

You can get an IRR ID from DEERS
