You will only be reimbursed if your local facility made the referral to the appointment at another installation. If you were referred there by your current MTF, you will submit a travel voucher with all receipts.

If you made the appointment at another installation and was not made by your current MTF, you will not be reimbursed.

If the hospital on your base regularly sends patients to the other hospital then there should be a shuttle that runs between the two. You should be able to do a Google search for a shuttle bus between your base and the other one. I did a quick search for Ft Hood and Ft Sam and the info came right up. Good luck.
