Has anybody on here dealt with a dependency discharge?

If so could you please share a checklist of some sort or the forms needed/how the process works.
I attempted to apply for it in the past but nobody could point me in the right direction so I ended up doing a compassionate reassignment instead.
This reassignment was an attempt to fix a temporary problem that has turned permenant so I wish to apply for the discharge.
Any Input is greatly appreciated.

I have a Hardship Dependency discharge. Make sure to have several affidavits and if it involves custody make sure to verify sole custody before application. I am currently dealing with this so I am just warning you make sure it is specified in Reason For Characterization on your Discharge and DA 4187 as well as what is causing specific dependency. Make sure to verify a solution, you are only one able to alleviate, describe what you have exhausted to come to this point. Make sure that you prove permanency rather than temporary.

I believe what you are looking for is considered a hardship discharge. At 635-200 ch 6 para 6-3

Roger SGT that is what I am looking for. Just trying to figure out what form this time of application goes on as in what DA Form.

Your S1 should be able to help you with the packet. Para 6-7 outlines the information you will need for the different reasons. Have you gone through your chain of command to see if they can assist? That would be my first recommendation if for no other reason than to plead your case, they may also have resources to help you. Good luck!!

SPC Taylor, according to the AR 635-200 that SGT Taylor-Campion referenced, you must do an application for separation. With that being said, the paperwork will be much like that of a compassionate reassignment. You will need the application, supporting documentation, and a DA 4187. Please do yourself a favor and familiarize yourself with Chapter 6 of that regulation. Good Luck!
