This was accidental. How could such an "accident" occur? Where does the responsibility lie? Some three-star or a sergeant in the Mail Room? How can a mistake of this magnitude be allowed to happen? What are the implications?

Maybe they should consider something as simple as color-coding the vials with a band or diffrent stopper to segregate extremely hazardous from merely hazardous. This shouldn't be rocket science to these people.
