Back pay? No. There is a thing called medical TDY and the unit can fund a person to drive a Soldier or dependent to a long distance appointment.
The unit can grant you use of the TMP to drive the Soldier. There is absolutely no reason why you should be using your own gas and car to drive someone else to their appointment and if you are that's a discussion you need to have with your 1SG.
As for the driving part, you can be tasked to drive whatever vehicle the unit has for you to drive

SFC (Join to see) - Wait so now I'm reading the post under the question - did an admin change the question? The question says taking dependents off post 'hours" away - but then the text says "due to his health issues he can't drive so I had to drive him to appointments thats hrs away. Can you get back paid for this?"
So now I'm confused. Because I've been a NMA for my daughter taking her to Omaha from Riley. And then IACH says on their page for a NMA attendant for an AD servicemember that the unit has to assign and fund the NMA if no family is available.
So you'd be right if he is driving another soldier to an off post referral over 100 miles away that the unit has to provide TMP.

SFC Kelly Fuerhoff I could swear that the heading of the question was different last night when I read it and answered it. I know sometimes the afmins come and change the questions to make them understandable. I don't remember anything about dependents in the original post lol

SGM Steve Wettstein Yes. I posted the chapter in the regulation on a separate comment on the thread that explains how the NMA works.

What back pay, your salary is set. If you are looking for travel type pay or support SFC Boyd laid it out perfectly.

The OP is trying to find out if he can be retroactively paid for being a non-medical attendant (NMA) for their dependent for off post appointments that are farther away. I don't know about retroactively but they need to go through his kid's PCM and the MTF to find out how they deal with NMAs. They probably can't get "back paid" for the past appointments if they were over 100 miles away - but they could be set up to get paid for any future appointments that are 100 miles or more (one way).
I have done NMA and got paid for the month my daughter was admitted and I had to drive from Ft Riley to Omaha (which is over 200 miles one way).

Just to ask is he serving or a veteran receiving health care at a VA facility.
Back pay for travel mileage is generally never paid as your have to claim it at time of travel at the VA facility. If it’s at a civilian medial faculty you have to file mileage reimbursement by mail or online. Additionally if you are speaking of “your caregiver back pay for driving” you’ll have to speak to transportation as that’s not usually back paid.
I hope this helps
