I'm trying to get National Guard plates for my vehicle, and my state requires a Tag-50 form. I have never heard of such a thing, nor where to find it. Any help is much appreciated.

I would think that it is something you can get from your dmv. I know in my state there are links on the dmv's website for things like this.

A. Each year your State HQ sends out a by name list of NG Soldiers in good standing and their current HOR. Your name is on that list.
B. A CDR memo from your unit stating you are a member in good standing and is entitled to a NG license plate.
