I for one think that we do our best to prevent all failure from happing as much as we can. After all we are only human. Tell me what you think?

Absolutely correct. Not sure that all things can be avoided but is the way life goes...

Always hate to see that, but these things happen in every dangerous job; military operations are inherently dangerous in peace time as well as in war.
