Looking for a 1SG spot with a medical TPU.

Doesn't the USAR centrally select 1SGs using eligible candidates from the MSG OML? I believe they switched to that a few years ago.

MSG (Join to see) - I ASSUME that the 1SG vacancies shown in the search tool* are ones that cannot be filled through the selection board. Either that or they are just in the database, but you have to go through the centralized selection process. Either way, as you said, it doesn't hurt to ask.
Doing a vacancy search and then a little 'results hacking'*, I came up with 50 x 1SG TPU positions that are vacant right now and are either for "AMEDD Enlisted" or are branch immaterial spots.
Additionally, the USARC G1 posts a vacancy list on MilSuite* in an Excel format.
Finally, work with the regional ARCC* and have them run down any leads or UICs you're interested in.

* "Results Hacking" ... search for your applicable Rank and click on any state that shows up in the map (for example, Utah shows 34 MSG positions vacant) - that brings up the spreadsheet for those positions.
Delete the "State" entry and click "Reload". That will show all vacant MSG positions in the USAR (1,692 at the time I ran it).
If you try to export the Spreadsheet, it will only show the vacancies in the state, so open a spreadsheet and copy and paste the entire results into it. Adjust fonts to your desired size for readability.
Filter the "Title" column to "FIRST SERGEANT" (there was one positing in UT that had theirs listed as "FIRST SGT" so I selected that as well). Filter the "Auth Br" to 'AE' and 'OT' ('AMEDD Enlisted' and 'Other Specialties').
