I have noticed my initial entry date if wrong, tried to fix it multiple times throughout my career, there are also more deficiencies in my DA 5016. When I was waiting for my MEB/ETS no one had done a DA 1506 to verify what it said on the computer. While I was out processing transition had org-day and I only had 4 hours to clear and unfortunately, my final out was the last thing on their mind, but while I was about to get my DD-214 I brought up the entry date being wrong the transition lady said I need to prove it, which I did with my initial contract. She amended the orders and told me that it will be fix. While on terminal leave almost two months later the entry date was still was not corrected, I brought it up to the retirement office because I was informed that they can help out and can do the DA 1506. Unfortunately, the lady said no to helping me out. That's when I submitted an ombudsman ticket and then I finally received a call from fort sill finance director on Dec02, 2022, she admitted to me on that phone call that a DA 1506 was not done to calculate my service timeline and that one I had from 2019 was not valid anymore because the form had updated in 2022. I need a DA 1506 done because I have been Active duty, National Guard, Active duty, Reserve, and last Active duty, all together 24 years of service, I admit I have two bad years from my National Guard time.
The fort sill finance director said to me that she would have a retention NCO do a DA 1506, a month after I was out the Army around late January 2023, I was CC on an email from the finance director asking the Retention NCO about the DA 1506, apparently Retention did have it done in Dec of 2022 but when Retention submitted the form to the retirement office and then retirement emailed it to finance. Finance would not process it without the DA 5016. I didn't have a copy, but the finance director got a copy in February of 2023, and I was told that Retention would verify the DA 1516 they had done in Dec of 2022 with my DA 5016. I haven't heard anything from fort sill. After reviewing the DA 5016 I noticed that there are some dates missing or incorrect. How can I get the DA FORM 5016 (CHRONOLOGICAL STATEMENT OF RETIREMENT POINTS) corrected?

From your description, I assume you're a gray area retiree. As you've tried through the administrative side and weren't able to get it fixed, submit it to HRC as your next step. If that doesn't work for some reason, then your last resort is to submit a request to the Army Board for Correction of Military Records (ABCMR).

For the HRC route, you must submit a request for correction of your retirement points, to include supporting documentation, to the HRC Personnel Action Branch's Retirement Points Team.*

The ABCMR route is the option when nothing else works (you must exhausts all other viable options or the packet is returned without action). You can find details on the ABCMR at https://arba.army.pentagon.mil/abcmr-overview.html.

COL Randall C. Thank you Sir for the response, yes, I verified with the Career Counselor this morning and she stand with the DA Form 1506 she calculated back in December 2022. She has me at 13 years 9 months December 2017 and didn't add the 5 years when I MEB/ETS on December 2022 because when she filled out the 1506, I was still in. If the 5 years is added to what she has calculated that would have put me at 18 years and 9 months, but Fort Sill finance didn't process it. I just called and left a message to the HRC Separation Documents Team. Once again thank you Sir.

SSG Dennis Mendoza - My apologies - I wrongly interpreted what you wrote as you had retired in the reserve component (need more coffee). However, the PAB Retirement Points Team is still the correct contact at HRC.

COL Randall C. No problem, Sir my timeline is very confusing I was Active duty, Guard, Active duty, Reserve, and last Active duty. Which that was what I'm referring to that the Fort Sill transition didn't get a 1506 done but I got it done and the Career Counselor certified/signed it, now it just to get someone to answer their phone.
