It's possible but it is case by case basis because it depends on the individual diagnosis and any additional details that may vary from person to person.

SGT (Join to see) - I understand. Mental Health like anything else is on a spectrum. I was on Active Duty with TS/SCI and while I was also diagnosed with PTSD and I served with plenty others who had various mental health conditions. I was also a DOD civilian with a Secret while receiving 50% from the VA for my PTSD. It's not an immediate disqualifying factor by any means. It generally doesn't come up unless there is a reason for it to be brought up.

Depends on the mental health issue. The investigator is looking for ways that you could be exploited, or become a liability. If you are medicated and stable and have a history that reflects that, you should be fine. Mental health professionals can speak on your behalf in a clearance investigation.
