Is "evidence of injustice" required for a retroactive Combat Action Badge?

I am working on putting several Soldiers in for a CAB they should have received for actions on their last deployment. The circumstances of the mission met the requirements for the award of the CAB, but they were not awarded the Badge.
The HRC website states " Requests for retroactive award of the CAB will not be made, except where evidence of injustice is presented. "
Would the Soldiers not receiving an award they were qualified for count as injustice in this case?

Not sure about the Army but Air Force has a lot of civilian CV and CD, in fact with CC deploying lot of time, the Civilian is acting CC. Gives institutional memory as CCs come and go.
In fact and indeed, last century when a Plans Officer with DCSIM 6th US Army PSF, I had a GS 15 as rater. Same - same as a Plans Officer DCSIM USASOC at Bragg. GS-15 also.

An explanation of why it wasn't awarded at the time. Lost paperwork, blown up computers, a reason for why they didnt put it in besides, "oh we just remembered"
