How come there is payscale for an 0-8 with less than 2 years of service?

If you google USPHSCC, you’ll find what you’re looking for. But essentially, it’s only commissioned officers, and they all receive direct commissions of varying grades, depending on the position.

It’s also common for docs with a lot of experience to commission at a higher grade than O-3. I met an O-6 that had less than a year of military service once.

Political appointees with no prior military service. Usually a wartime situation. Do not believe any exists today. I know of a O-5 with less than two years of service. Football great Otto Graham became an instant full commander as coach at the US Coast Guard Academy. Latter promoted to Captain (O-6). CGA did this to give him the pay and benefits that a usual football coach did not get.

Image commissioning as an O-8 Doc or something and having to wait 20 years to get a promotion. Probably has never happened, but still funny to think about.
