How can I contact the 201st Regional Training Institute for a copy of my 1059?

I attended BLC, then WLC at the 201st Regional Training Institute, Juana Diaz- Puerto Rico around December 2012. I have been looking all over the net to try to find a contact number to contact them for a copy of my 1059, evaluation report with no success. It was not uploaded into my iperms. Will anybody know or have a contact number for the school house there? Any help will be greatly appreciated. Thanks.

Call this number [login to see] extension 8037 (Ops and Training). You may have to provide the dates of attendance and your Class Number.

Your training NCO or S-1 should have access to ATTRS and should be able to pull that you have completed WLC. I'm not positive on the 1059 but they should be able to show completed through ATTRS.
