How do we fix the issues of properly tracking sex offenders?

I was searching through the internet and came across this article highlighting a breakdown in sex offender management. There needs to be a policy change or something implemented to keep these individuals monitored, tracked, and closely watched so things (mentioned in the article) won't happen.

How do we fix it? I guess by better coordination between the military and civilian authorities. As the Congresswoman said, this must be fixed.
I would say that when someone leaves the military as what would be considered a sex offender in civilian life, there should be a system in place that transfers that information to civilian authorities and requires the individual - under penalty of incarceration for not complying - to register as a sex offender. Slipping through loopholes like this is unacceptable.
