I know this might not answer your question directly, I believe your best course of action will be to go through your unit S-1.

No matter how many examples you find here (or on S1NET, and other sites), each installation's Defense Military Pay Office (DMPO) traditionally has "their way" of doing things.

Your unit S-1 will be in the best position to already know how to both (a) prepare and process this action; and (b) interact with your local DMPO to ensure that the action is processed in a timely manner.
