The last PT test I took was 27 October, on the 28th I was playing soccer and tore all of the ligaments in my ankle. I've been on temporary profile ever since because it was supposed to "heal itself" with physical therapy. Now 9 months later I am getting surgery and it may take 3-6 months to heal which would put my PT test as over a year old. Will this take away my promotable status?

Once your APFT expires on your PPW, you will lose your status on the C10 roster. Even if it is a medical reason. You're expected to maintain yourself and your health. The only way a medical reason keeps a promotable status is if you were enrolled in a med board (MEB). Then your promotable status cannot be touched for the duration of the MEB.

Torn ligaments have to be surgically repaired. I'd run from whoever told you they'd heal on their own.

Some can just depending on how bad it is, but in my case it didn't heal on its own.

Not sure he can run with torn ligaments. I'd suggest hobbling.

I'd say no based on my sons experience. Coming out of Afghanastan he suffered from many problems the most serious of which was a respiratory problem which has been improving greatly but he was put on profile for asthma. He's since changed duty stations and was promoted to Tech Sgt in 7 years which is well ahead of schedule. He's currently UDM at Lewis/McChord, and hopefully awaiting orders to the 160th SOAR. His only requirement is he must pass the Army PT test, that's it.
