Has anyone submitted a successful MOS Conversion Bonus Packet?

I am building a packet for a Service Member that is transferring from the IRR to the SELRES and they are eligible to receive the MOSCB, but the examples we have seem to be incomplete. I am trying to find a completed example of a successful request so I am not wasting time trying to get it right.

Cant you find them a will train position for the MOS they are looking at for the MOSCB? If not, contact MSG Erica Lomax, she should be on the global. She is the NCOIC for the USARC incentives team and is the POC for the MOSCB. She can assist you with the process.
