Was it done on a 638? I am being told this local MOVSM recommendation form is all that is needed but I thought I would need a 638 with orders.

I did one off the HRC memo, exactly as shown.
S-1's don't know what to do with it, simply because it's new. But...it's straight off HRC, it's not complicated, it just needs your 1SG and LTC or higher's signature.
Just fill in the blank. What more do you need to know?

It's new, before you could just simply submit a 638, now the MOSVM is approved via memorandum. No one has submitted on in my unit yet so they don't have an example
