All I can do is give my example. My filing for my claim, I included a statement with my DD-214 Re-entry code (4K, Medical Disqualification from Continued Active Service), so that it would be in two places (my DD-214 and statement) that my continued ability to serve was directly impacted by my medical conditions. You may also want to include the separation code if it shows that you were medically discharged, separated earlier than your contract, or such.

As always, the best advice I can give you is to go see a VSO at your local VA facility. They can tell you better what you should be putting into your claim.
