This past year I was going through my records and noticed that my DD214 was missing a few medals. So I submitted a DD149 along with supporting statements and the award forms. Months later, I received a letter from the BCMR that said I was successful. I have included a copy of my documents to show you exactly what to do when writing your paper and submitting evidence when writing to the Army Board of Corrections of Military Records (BCMR).
