As both a former Air Force Safety Officer and Flight Scheduler there are more errors than I can count swirling around this incident. Maj Robert Thornton is absolutely correct.

a truly avoidable incident. so many things that should never have happened. hopefully, commanders learned a valuable lesson about going by the book
