There is still usually someone at fault in accidents.
If someone pencil-whipped a safety inspection and blessed off on faulty safety equipment or wiring or relays or whatever, that would be wrong-doing - which led to an accident. And it would not be *sabotage*, just negligence.
If the captain meant to turn left, but accidentally turned right, that would be an accident - and also wrong-doing.
Etc.
I know you want Biden to be the bad guy in EVERY story, but it just ain't so. At least not always.
