Anyone here who is a leader in the military or in the civilian world should read this story and learn valuable lessons on problem-solving and a lessons of dealing with management and bureaucracy. This also show that a product, like this MK-14 torpedo, not properly tested for mechanical defects, can have a catastrophic outcome.
The USN did not have an updated doctrine to work with the new technology. This lack of proper doctrine and decentralized navy leadership in Washington blaming the Submarine crews instead of taking a look at a defective torpedo delayed fixes and probably cost a few submarines to be sunk by the Japanese resulting in lives lost for nothing.

I saw a John Wayne move about these faulty torpedo's. Some of the BS our senior leaders will deny or cover up to cover their asses. If there's a problem get everyone involved and fix it. How many men had to die before they excepted there was a problem? Stop trying to lay the blame just fix the damn problem.
