Nowadays unions seem to increasingly represent the higher paid groups - such as the NFL, utility company employees and civil servants - who don't actually need unions.

If you are a patient and need treatment to remain healthy, you can't smoke on site and make hazardous health conditions for other patients
If you are one of the people helping patients remain healthy, you are allowed to smoke on site and create conditions which will likely harm the patients you are tasks to help....
